Key Responsibilities- Provide direct patient care, including pain and symptom management, to ensure optimal comfort and well-being.
- Implement nursing practice based on comprehensive assessments and care plans, ensuring patient-centered care.
- Document patient services accurately and timely, maintaining high standards of record-keeping.
- Assess and educate patients and their families/caregivers on various aspects of care, promoting empowerment and understanding.
- Participate in the coordination of hospice services, ensuring seamless care delivery.
- Manage pain and symptoms effectively, evaluating patient responses to treatments and medications.
- Engage in professional growth and development opportunities, staying up-to-date with best practices and quality assessment activities.
- Ensure compliance with state and federal regulations, upholding the highest standards of care.
- Graduate of an accredited practical or vocational nursing program, with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in community health, hospice, or med/surg settings is highly valued.
- Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
