Family Service Care Coordinator Role Description:

This position requires a highly motivated and detail-oriented individual to provide case management and care coordination services to children, adolescents, and families. The ideal candidate will have a strong passion for working with diverse populations and a commitment to delivering high-quality services.

Main Responsibilities:
  • Conduct thorough assessments of assigned clients to identify their needs and develop personalized service plans.
  • Offer community, school, and home visitation services, as well as instruction in adult daily living skills, behavior management, stress management, and parenting techniques.
  • Prepare and submit emergency funding, housing, utility assistance, medical, and state benefits applications for clients as needed.
  • Maintain accurate and up-to-date record keeping for client files, adhering to program standards and accreditation requirements.
  • Utilize computer database systems to track client information and complete required paperwork.
Requirements:
  • Bachelor's degree in Social Work (BSW), Bachelor of Arts (BA), or Associate's degree (AA) and/or related experience.
  • Demonstrated case management skills and experience.
  • Familiarity with child development, emergency intervention methods, family dynamics, health, and mental healthcare systems.
  • Case management certification or other certification required by the program or willingness to obtain it.
  • TB test, valid driver's license, proof of liability insurance, and transportation.
  • Excellent oral and written communication skills, with bi-lingual Spanish a plus.
Working Conditions:

This role requires flexibility with scheduling to accommodate clients' needs, occasional travel to CCAP locations, and work in schools, clients' homes, and the community.

Estimated Salary:

$19-$20 per hour, based on industry standards and market research.
