As a Per Diem Diabetes Educator, you will work closely with the inpatient Diabetes Care Team at Wentworth-Douglass, a recently voted Best Place to Work on The Seacoast and a recognized Magnet Hospital.

You will provide Diabetes Education and Care to Patients and Staff in a variety of settings, including ED, Inpatient, Critical Care, and Same Day Surgery.

Collaborate with engaged providers and hospitalist team to deliver quality care for optimal outcomes.

Work with committed clinical staff to assist in discharge planning and continuum of care.

Responsibilities
  1. Assesses, plans, implements and evaluates learning needs of the inpatient and outpatient population.
  2. Provides inpatient programs for nursing personnel.
  3. Evaluates and revises as needed all resource materials for diabetes education for nursing staff.
  4. Utilizes research as a basis for providing and improving diabetes education.
  5. Participates in Care Team Rounds.
  6. Consults with nursing staff as requested for inpatient referrals.
  7. Provides Diabetes Education to inpatients per assessed needs.
  8. Provides individual and group outpatient Diabetes Education.
Requirements
  • Minimum 3 years experience in an acute care hospital; direct patient diabetes education experience.
  • Diabetes Education Certification within 3 years from date in job.
  • Registered Nurse License.
