Key Responsibilities- Provide direct patient care in post-acute facilities, including administering medications, monitoring vital signs, and conducting intakes and documentation.
- Collaborate with healthcare teams to develop and implement resident care plans.
- Conduct regular assessments of resident wellbeing and report any concerns to facility staff.
- Participate in IV set up, immunizations, and phlebotomy procedures as needed.
- Assist with the orientation and training of new medical staff.
- Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN).
- Completion of an approved Licensed Practical Nurse (LPN) educational program.
- High school graduation, GED, or equivalent.
- At least 6 months of experience as an LPN or in a similar role.
- Ability to lift a minimum of 50 lbs.
- Competitive hourly pay rate of $32-$65.
