Job Summary

We are seeking a skilled Speech Therapist to join our team of healthcare professionals. As a Speech Therapist, you will be responsible for providing speech therapy services to patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that address the unique needs of each patient.

Responsibilities
  • Evaluate patients' level of functioning and hearing, and recommend mechanisms to enhance their hearing ability.
  • Provide initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Establish speech therapy treatment plans in consultation with physicians and revise them as necessary.
  • Assist in the development and implementation of interdisciplinary client care plans as they pertain to speech therapy.
  • Communicate with other team members and instruct them in speech pathology techniques as needed.
  • Teach, supervise, and counsel patients and their families in the total speech therapy program and other related problems.
  • Record all evaluation data, treatments, and patients' responses to therapeutic intervention in a timely manner.
  • Supervise Home Health Aides every 14 days when speech therapy is the only professional discipline involved in patients' care.
  • Record and report patients' reactions to treatment and any changes in their condition to physicians.
  • Participate in the development of periodic revisions to physicians' plans of treatment, keeping them informed of patients' status and obtaining additional orders as necessary.
  • Communicate with physicians at least every 30 days or whenever changes occur regarding patients' status and obtain additional orders as needed.
  • Evaluate the effectiveness of speech pathology intervention and update client care plans as needed.
Qualifications
  • Graduate of an accredited speech therapy program or equivalent education.
  • Licensed in the State of Florida as a Speech Therapist.
  • Two years of clinical experience as a Speech Therapist.
  • Current BLS Certification.
  • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
  • Must display good emotional health and be able to physically tolerate much standing, stooping, and bending; must meet all agency regulations.
  • Must be self-directed and able to work with little supervision and have good observational, judgment, and communication skills.
