The organization is currently expanding its team in the Cincinnati area and will collaborate with providers to establish a suitable geographic territory for patient care.

COMPANY PROFILE:

This organization is committed to delivering accessible, high-quality primary care to homebound and facility-based geriatric patients. With a dedicated team of over 15 healthcare professionals, including physicians and nurse practitioners, they ensure that patients receive exceptional medical care without the need for travel. Their services encompass primary care, chronic care management, psychiatric care, post-acute care, transitional care, and remote patient monitoring.

POSITION DESCRIPTION:

The nurse practitioner will act as the primary care provider for a designated group of geriatric patients. All patients are situated in facilities or private homes. Depending on the assigned territory, the practice can offer various combinations of facilities and home-based patients. During the initial interview, the organization will discuss the provider's location and preferences in detail.

In skilled nursing facility settings, providers typically see an average of 15 to 16 patients daily. When traveling between assisted living, independent living, and home settings, the average is 9 to 12 patients per day, adjusted for travel time.

Key Responsibilities:
  • Conduct comprehensive health assessments to identify patient needs
  • Develop individualized care plans
  • Regularly assess patient conditions and modify treatment plans as necessary
  • Administer medications, treatments, and other therapeutic interventions as prescribed
  • Perform minor procedures and wound care as required
  • Monitor vital signs and patient responses to treatment
  • Educate patients and families on health management, disease prevention, and treatments
  • Provide guidance on lifestyle modifications and self-care practices
  • Collaborate with physicians, nurses, and other professionals to coordinate comprehensive care
  • Ensure compliance with all relevant healthcare regulations and standards

The practice utilizes the eClinicalWorks EHR system.

On-call responsibilities involve phone triage only, with a low volume of calls. Providers share on-call duties, rotating every 10 weeks or less for full-time staff. Part-time providers will participate in on-call duties based on a negotiated schedule.

  • Minimum of 1 year of advanced practice experience
  • Previous experience in geriatrics, long-term care, skilled nursing facilities, or house calls
