Job Description

Job Summary:

Vita Nova, Inc. is seeking a dedicated and compassionate Housing Case Manager to join our team. As a key member of our youth housing program, you will provide individualized case management services, innovative programming, and principles of positive youth development to assist former Foster Care, LGBTQ, and other disconnected homeless youth in achieving stable and safe housing.

Key Responsibilities:

  • Case Management: Maintain accurate and up-to-date documentation and reporting standards, including tracking census, program eligibility, participation, and challenges in the homeless management information system (HMIS).
  • Life Skills Training: Facilitate weekly individual and group Life Skills training in the home and community, focusing on essential skills such as time management, budgeting, and conflict resolution.
  • Service Coordination: Administer the TAY-VI-SPDAT vulnerability index and service prioritization to homeless youth, ensuring they receive necessary services and support.
  • Education and Employment: Educate participants on landlord/tenant rights and responsibilities, lease agreements, pest control, and apartment maintenance, as well as provide ongoing employability, education, and career planning skill building.
  • Community Partnerships: Partner with local organizations and service providers to connect youth to needed services, including mental health, physical/dental health services, substance abuse, domestic violence, and social supports.
  • Team Collaboration: Coordinate staffing and team meetings to assess progress and develop ongoing support teams, ensuring a comprehensive and supportive environment for youth.
  • Additional Responsibilities: Recruit, train, and orient volunteers and mentors serving as permanent connections, provide transportation and community-based services in partnership with the youth participant, and attend community collaboration and networking meetings to expand services offered to youth.

Requirements:

  • Education: Bachelor's degree in social work, mental health counseling, or other human service-related field. Master's degree and experience in mental health are preferred.
  • Skills and Experience: Understand and implement trauma-informed care practices and a positive youth development framework, possess critical thinking skills and the ability to assess crises and create safety plans, and be self-directed and able to create Life Skills curriculum and lesson plans.
  • Personal Qualities: Ability to work non-traditional hours, including evenings and weekends, and be on-call to address roommate conflicts or resolve crises in the home.
