Caledonia Senior Living & Memory Care - OVERVIEW: The Practical Nursing Specialist is tasked with delivering direct nursing services to residents while overseeing the daily nursing operations conducted by nursing aides.

This oversight must align with the prevailing federal, state, and local standards, guidelines, and regulations governing our facility, as directed by the Director of Nursing, to ensure the utmost quality of care is consistently upheld.


KEY RESPONSIBILITIES:
1. Supervise the daily activities of nursing aides.
2. Collaborate with assigned nursing staff and support personnel to plan the services, programs, and activities for the shift.
3. Prepare written and verbal reports or recommendations regarding shift activities as necessary.
4. Manage the admission, transfer, and discharge processes for residents.
5. Provide guidance to nursing staff assigned to your unit or shift.

6. Complete and maintain necessary documentation forms and charts upon resident admission, transfer, or discharge.
7. Receive and transcribe physician orders, recording them on the Physician's Order Form.

8. Document nursing notes in a clear and descriptive manner that reflects the care delivered and the resident's response.

9. Fill out and submit accident or incident reports to the Director as required.
10. Conduct routine charting duties in accordance with established policies and procedures.

11. Prepare and administer medications as prescribed by the physician.


12. Foster and maintain positive working relationships with inter-departmental personnel and other facility departments to ensure nursing services meet resident needs.

13. Create and sustain an environment of warmth, personal interest, and positivity, while ensuring a calm atmosphere throughout the unit and during shifts.

14. Exchange nursing reports at the beginning and end of shifts.
15. Accompany physicians on rounds as necessary.

PROFESSIONAL SERVICES INCLUDE:

catheterization, tube feedings, suctioning, applying and changing dressings, managing colostomy and drainage bags, performing blood draws, providing massages, and conducting range of motion exercises, as required.


16. Monitor assigned personnel to ensure compliance with established safety regulations regarding equipment and supplies.

17. Uphold confidentiality of all resident care information, including protected health information, and report any known or suspected breaches of confidentiality.

18. Ensure that all nursing care is delivered in privacy, and that nursing staff knock before entering resident rooms.

19. Perform additional duties as assigned.

QUALIFICATIONS:
To excel in this role, candidates must successfully fulfill each essential duty. The following qualifications are essential:

1. Licensed Practical Nurse with a current, unencumbered state license.

2. Proficient in reading, writing, speaking, and understanding the English language.

3. Ability to make independent decisions when circumstances necessitate.


4. Knowledgeable in nursing and medical practices, procedures, and relevant laws, regulations, and guidelines pertaining to nursing care facilities.

5. Current CPR Certification is mandatory.

6. Experience in Long Term Care is preferred.

3. Must meet the general health requirements established by facility policies, including medical and physical examinations.


4. Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a height of at least 5 feet and carry such weight a minimum distance of 50 feet.

5. May be required to assist in the evacuation of residents during emergency situations.
