As a Home Health Occupational Therapist, your responsibilities will include:
- Evaluating and screening patients' daily living and work-related skills, while creating therapeutic retraining programs with measurable goals.
- Administering and interpreting diagnostic tests to assist physicians in assessing patients' functional levels in their homes or other environments.
- Collaborating with physicians and healthcare team members to develop and update the Plan of Care Treatment.
- Delivering therapeutic interventions and guidance to patients based on physician directives to enhance strength, coordination, range of motion, and overall function.
- Adjusting retraining programs to align with each patient's behavioral objectives and the overarching rehabilitation guidelines.
- Consulting with vocational team members to create and implement plans that cater to patients' needs and capabilities.
- Documenting patient progress and status to physicians, rehabilitation staff, Clinical Managers, and family members.
- Engaging in care coordination and discharge planning, serving as a resource for other healthcare professionals in addressing patient needs.
- Designing community reintegration activities to aid clients in their physical and psychological adjustments, coordinating efforts with the interdisciplinary team.
- Recommending or creating specialized adaptive equipment to enhance clients' living or working environments and support their rehabilitation journey.
- Training staff and family members to assist in executing the retraining program, including essential daily living skills.
- Overseeing and evaluating the performance of Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.
- Ensuring thorough documentation of patient care observations, interventions, and evaluations on the day services are provided, including accurate implementation of interim physician orders.
Essential Qualifications:
- Degree from an accredited Occupational Therapy Program.
- Preferred experience of at least six months in occupational therapy.
- Home health experience is advantageous.
- Current and valid OT licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation are mandatory.
Work Schedule: 40 hours per week
Compensation: The salary range for this position is between $76,800 and $105,800 annually, reflecting a good faith estimate of starting base pay for full-time employment. Actual pay may vary based on geographic location and individual qualifications.
