About the Role
CenterWell Home Health is seeking a skilled Occupational Therapist to join our team. As a Home Health Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or surgery and regain their independence.
Key Responsibilities
- Conduct comprehensive assessments of patients' daily living and work-related skills to develop personalized rehabilitation plans.
- Administer and interpret diagnostic and prognostic tests to evaluate patients' level of function and inform treatment decisions.
- Collaborate with patients' physicians and other healthcare team members to develop and revise treatment plans.
- Provide therapeutic treatment and instruction to patients to improve strength, coordination, range-of-motion, and function, or teach compensation measures.
- Review and expand rehabilitation plans to ensure they align with patients' behavioral goals and the overall rehabilitation program.
- Consult with other vocational team members to develop and implement vocationally oriented plans that meet patients' needs and capabilities.
- Communicate patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
- Participate in care coordination and discharge planning activities and serve as a resource to other healthcare personnel in meeting patients' needs.
- Design community reintegration activities to assist patients in physical reconditioning and psychological adjustment, and coordinate these plans with the interdisciplinary team.
- Recommend and design special adaptive equipment to improve patients' residence or working environments or enhance their participation in the rehabilitation program and community.
- Train staff and family members to carry out the retraining program, including dressing, feeding, grooming, and hygiene skills, and participate in team and family educational meetings.
- Supervise, instruct, and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
- Accurately and thoroughly document patients' care observations, interventions, and evaluation on the day services are rendered, and ensure that interim orders received from physicians are accurately documented and implemented.
Requirements
- Degree from an accredited Occupational Therapy Program.
- A minimum of six months of occupational therapy experience preferred.
- Home Health experience a plus.
- Current and unrestricted OT licensure.
- Current CPR certification.
- Good organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation are required.
