About the Role

We are seeking a skilled Primary Care Nurse to join our team at Providence Transitional Care Center. As a Primary Care Nurse, you will be responsible for providing high-quality patient care to a specific group of residents, including supervision of team members, coordinating referrals to other healthcare professionals, and serving as the point of contact for resident information.

Key Responsibilities
  • Supervise and coordinate the care of residents, ensuring that their needs are met and their well-being is maintained.
  • Collaborate with the interdisciplinary healthcare team to develop and implement individualized care plans.
  • Provide education and support to residents and their families on health-related topics and disease management.
  • Assist with clinical interventions and resident activities of daily living.
Requirements
  • Graduate of an accredited School of Nursing.
  • Alaska Registered Nurse License.
  • National Provider BLS - American Heart Association.
Preferred Qualifications
  • 6 months Nursing experience in a healthcare facility (acute care, long term care, rehabilitation center).
  • Supervision/leadership experience.
