Your Role: Hospice Aide / Certified Nursing Assistant (CNA)

As a Hospice Aide / CNA, you will provide essential care and support in the sacred space of a patient's final journey, becoming a key member of our esteemed healthcare team. Your responsibilities will include:

  • Collaborating with a registered nurse and other clinicians to deliver care according to the service plan.
  • Providing companionship and personalized care, encouraging independence and self-care in clients.
  • Assisting clients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
  • Supporting clients with the self-administration of medication as needed, ensuring safety and adherence to protocols.
  • Providing mobility and transfer assistance within guidelines, promoting patient safety and comfort.
  • Preparing meals and snacks for clients, which may include grocery shopping according to dietary needs and preferences.
  • Performing light housekeeping duties such as vacuuming, laundry, and linen changes to maintain a clean and comfortable environment.
  • Assisting with incontinence care, ensuring client dignity and hygiene.
  • Supporting clients with transportation needs for appointments, errands, or outings.
  • Remaining in compliance with HIPAA guidelines and all state and federal regulations. Documenting clinical notes clearly and concisely.
  • Setting up and maintaining medical equipment and supplies, including Durable Medical Equipment (DME), ensuring readiness and functionality.
What We're Looking For
  • Valid CNA certification in the State of Pennsylvania.
  • Experience in hospice or palliative care preferred.
  • Clear background check and drug screening.
  • Strong communication and empathy skills.
  • Self-starter, able to work independently and collaboratively.
  • Physical ability to perform job duties.
  • Reliable transportation and a valid driver's license.
