A Medication Support Specialist plays a crucial role in assisting individuals facing mental health challenges within their homes and communities. Responsibilities include traveling to clients' residences to conduct medication observations and support daily living skills, ensuring adherence to prescribed medication regimens and promoting safety for independent living.
The primary objective of this position is to empower clients to achieve the highest possible level of independence, functionality, and mental well-being, enabling them to live safely both at home and within the community. This role emphasizes fostering independence in daily activities while nurturing interdependence in social relationships.
Key Responsibilities- Monitor and document clients as they take their prescribed medications, providing assistance as necessary to ensure proper intake.
- Educate and assist clients in essential daily living skills, including budgeting, meal planning, preparation, and maintaining a clean and organized living environment.
- Maintain accurate and timely documentation of services rendered to clients, adhering to established policies and procedures.
- Observe and report on clients' mental and physical health to the designated treatment team members and/or the Director of Home Care and Support Services.
- Recognize and respond appropriately to crisis situations and emergencies.
- Engage in regular caseload reviews with supervisors.
- Participate actively in team meetings, agency gatherings, and training sessions when available.
- Minimum age of 18 years.
- High school diploma or GED required.
- Valid driver's license, insured vehicle, and a clean driving record as determined by company policy.
- Ability to pass a criminal and caregiver background check.
- Strong time management abilities to complete assigned tasks within designated timeframes.
- Competence in accurately documenting client observations and task completion.
- Ability to communicate information to clients in a direct yet respectful manner.
- Effective interaction with clients, professionals, colleagues, and community members.
- Respect for client time through active engagement (e.g., no personal phone calls or distractions during client interactions).
- Capability to work independently without requiring constant supervision.
This position does not involve hands-on care. Evening hours, if necessary, can conclude by 7 PM. Accommodations for vacations and time off can be arranged, with other team members available to cover responsibilities during absences.
The starting wage for this position is $15.00 per hour, with potential for negotiation and increases based on relevant experience.
