Hospice Social Worker Job Summary

SSM Health is seeking a compassionate and skilled Hospice Social Worker to join our team. As a Hospice Social Worker, you will provide essential support to patients and their families, addressing social determinants of health and promoting overall well-being.

Key Responsibilities:
  • Conduct thorough assessments of patients' social needs and develop personalized plans to address these needs.
  • Collaborate with healthcare teams to ensure comprehensive care and support for patients and their families.
  • Connect patients and families with community resources and services to enhance their quality of life.
  • Develop and maintain relationships with community agencies and organizations to facilitate access to resources and services.
  • Participate in multidisciplinary team meetings to share knowledge and best practices.
Requirements:
  • Bachelor's degree in Social Work or related field.
  • No prior experience required, but a strong desire to make a difference in the lives of others.
