Position Overview:

Located in a collaborative environment, this position focuses on addressing social determinants of health that impact patient well-being.

Key Responsibilities:

  • Conduct interviews with patients to assess their needs and document case activities.
  • Collaborate with a multi-disciplinary team and connect patients with community resources to enhance their treatment.
  • Guide patients and families in accessing community assistance through appropriate referrals.
  • Establish and nurture relationships with local health, welfare, and social service agencies to creatively meet patient needs.
  • Actively engage with team members to identify opportunities for improving standard practices.
  • Maintain a vigilant and safe working environment.
  • Perform additional duties as required.

Qualifications:

Education:

  • Master's degree in Social Work is required.

Experience:

  • No prior experience is necessary.

Physical Demands:

  • Regularly engage in reaching, sitting, standing, and walking.
  • Utilize vision and depth perception for tasks at varying distances.
  • Communicate effectively using hearing and speech for oral exchanges.
  • Frequent keyboard usage and data entry tasks.
  • Occasional physical activities including bending, lifting, and driving.

Work Schedule:

This position operates on a day shift schedule.
