Position Overview:
Located in a collaborative environment, this position focuses on addressing social determinants of health that impact patient well-being.
Key Responsibilities:
- Conduct interviews with patients to assess their needs and document case activities.
- Collaborate with a multi-disciplinary team and connect patients with community resources to enhance their treatment.
- Guide patients and families in accessing community assistance through appropriate referrals.
- Establish and nurture relationships with local health, welfare, and social service agencies to creatively meet patient needs.
- Actively engage with team members to identify opportunities for improving standard practices.
- Maintain a vigilant and safe working environment.
- Perform additional duties as required.
Qualifications:
Education:
- Master's degree in Social Work is required.
Experience:
- No prior experience is necessary.
Physical Demands:
- Regularly engage in reaching, sitting, standing, and walking.
- Utilize vision and depth perception for tasks at varying distances.
- Communicate effectively using hearing and speech for oral exchanges.
- Frequent keyboard usage and data entry tasks.
- Occasional physical activities including bending, lifting, and driving.

This position operates on a day shift schedule.
