Job Summary

We are seeking a skilled Social Worker to join our Hospice team. As a Social Worker, you will provide emotional and practical support to patients and their families, helping them navigate the complexities of end-of-life care.

Key Responsibilities
  • Conduct thorough assessments of patients' social and emotional needs
  • Develop and implement personalized care plans to address these needs
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide emotional support and counseling to patients and their families
  • Connect patients with community resources and services to enhance their quality of life
Requirements
  • Master's degree in Social Work (MSW) or related field
  • Experience working in hospice or palliative care (not required but preferred)
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work varying shifts, including evenings and weekends
