Director of Case Management Job Summary

The Director of Case Management provides strategic leadership and oversight for the case management department, ensuring high-quality patient care and efficient resource allocation. This role requires a seasoned professional with expertise in case management, nursing, and leadership.

Key Responsibilities:
  • Develop and implement case management policies and procedures to ensure compliance with regulatory requirements and organizational standards.
  • Lead and manage a team of case managers, providing guidance, coaching, and performance feedback to ensure excellent patient outcomes and staff development.
  • Collaborate with interdisciplinary teams to develop and implement care plans that meet patient needs and optimize resource utilization.
  • Monitor and analyze departmental performance metrics, identifying areas for improvement and implementing strategies to enhance quality and efficiency.
  • Develop and manage budgets, ensuring effective resource allocation and fiscal responsibility.
  • Stay up-to-date with industry trends, best practices, and regulatory requirements, applying this knowledge to inform departmental decisions and strategies.
Requirements:
  • Master's degree in Nursing or related field (or equivalent experience).
  • Current licensure as a Registered Nurse in the State of Ohio.
  • Minimum 3 years of experience in case management, with a focus on leadership and management.
  • Proven track record of success in case management, with expertise in policy development, team leadership, and quality improvement.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including patients, families, and healthcare professionals.
