Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Peritoneal Dialysis Registered Nurse (PD RN). As a PD RN, you will play a vital role in providing education and support to patients and their care partners, empowering them to take control of their healthcare and manage their dialysis treatment independently.

Key Responsibilities
  • Provide comprehensive patient care in an independent setting, applying your core nursing skills to assess, diagnose, and implement complete care plans.
  • Empower patients by teaching them to administer their own dialysis and take ownership of their health.
  • Lead, coach, and guide patients, support systems, and care partners, building deep relationships and fostering a sense of community.
  • Collaborate with healthcare teams to ensure seamless care transitions and provide ongoing support to patients and their families.
Requirements
  • Current Registered Nurse (RN) license in the state of practice.
  • Minimum of 12 months of RN experience.
  • Basic computer skills and proficiency in MS Word and Outlook.
Preferred Qualifications
  • Previous experience in chronic hemodialysis, acute dialysis, home hemodialysis, or peritoneal dialysis nursing.
  • Training, teaching, or healthcare education background.
  • Case management experience.
  • Familiarity with telehealth technologies.
