The Mental Health Association in Orange County, Inc. is a private, not-for-profit organization dedicated to promoting the mental health and emotional well-being of Orange County residents.

We are seeking a skilled Care Management Specialist to join our team

Job Description

This role will involve providing care management services to individuals enrolled in our Health Home Plus Care Coordination Program, working primarily with those diagnosed with HIV and/or mental health issues.

The successful candidate will be responsible for:

  • Attending bi-weekly Care Coordination team meetings
  • Attending trainings and meetings relevant to Health Homes and Health Home Plus
  • Delivering care coordination services as outlined under the Health Home Plus Model for Care Coordination
  • Completing New York State Community Mental Health (CMH) assessments and reassessments
  • Submitting monthly billing for caseload
  • Completing monthly scorecard data collection information
  • Maintaining all client records electronically through FCM and AWARDS Electronic Record Keeping
  • Maintaining all client files according to program policies and procedures
  • Establishing and maintaining linkage with appropriate community agencies and service providers
  • Representing MHA at meetings as necessary

Requirements

To be considered for this position, you must have:

  • A Bachelor's Degree in any of the following: child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing; OR A bachelor's level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; OR A Credentialed Alcoholism and Substance Abuse Counselor (CASAC)
  • Two years of experience (a Master's degree in a related field may substitute for one year's experience)
  • Experience providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse; OR Linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse to a broad range of services essential to successfully living in a community setting
  • Bilingual Spanish/English skills
  • Effective written and oral communication skills
  • Experience with Health Homes highly preferred
  • Knowledge of Medicaid procedures highly preferred
  • Experience with Electronic Health Record Program(s) required
