Healthcare Specialist

We are seeking a skilled Healthcare Specialist to join our team in Middletown. In this role, you will provide care management to individuals enrolled in the Health Home Plus Care Coordination Program, working primarily with individuals with HIV and/or mental health issues.
Key Responsibilities- Attend bi-weekly Care Coordination team meetings to ensure seamless collaboration and effective communication.
- Participate in relevant trainings and meetings to stay updated on Health Homes and Health Home Plus best practices.
- Deliver high-quality care coordination services as outlined under the Health Home Plus Model for Care Coordination.
- Complete New York State Community Mental Health (CMH) assessments and reassessments to ensure comprehensive care.
- Submit accurate and timely billing for caseload to maintain program efficiency.
- Collect and maintain monthly scorecard data to track progress and identify areas for improvement.
- Maintain all client records electronically through Foothold Technology and FCM Electronic Record Keeping to ensure secure and compliant storage.
- Bachelor's Degree in a related field such as child and family studies, community mental health, counseling, education, nursing, occupational therapy, physical therapy, psychology, recreation/recreation therapy, rehabilitation, social work, sociology, or speech and hearing.
- NYS licensure and current registration as a Registered Nurse and a bachelor's degree; or a bachelor's level education or higher in any field with five years of experience working directly with persons with behavioral health diagnoses; or a Credentialed Alcoholism and Substance Abuse Counselor (CASAC).
- Two years of experience providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse; or linking persons who have been diagnosed with mental disabilities, developmental disabilities, alcoholism, or substance abuse to a broad range of services essential to successfully living in a community setting.
- Bilingual Spanish/English a plus.
- Effective written and oral communication skills.
- Experience with Health Homes highly preferred.
- Knowledge of Medicaid procedures highly preferred.
- Experience with Electronic Health Record Program(s) required.
