Job Summary

We are seeking a highly skilled Rehabilitation Services Coordinator to join our team at Middletown Park Rehabilitation and Healthcare.

About the Role

The successful candidate will be responsible for safely transporting residents to and from therapy sessions, assisting therapists with treatments, accompanying residents on outings, setting up and maintaining equipment used in the therapy process, performing light clerical tasks, and maintaining a clean and organized rehab department.

  • Transport residents to and from therapy sessions in a safe and efficient manner.
  • Assist therapists with treatments and accompany residents on trips out of the facility.
  • Set up and maintain equipment used in the therapy process and other areas.
  • Perform light clerical tasks as required and assist nurses and therapists with daily care and patient therapy.
  • Communicate resident performance, condition, and participation during delegated patient care activities.
  • Maintain the rehab department organized and free of clutter.

Requirements

To be successful in this role, you will need:

  • A minimum High School Diploma or equivalent.
  • Excellent communication and organizational skills.
  • The ability to work well under pressure and be flexible.
  • Exemplary customer service skills.
  • A current CNA certification is preferred but not mandatory.
  • The physical ability to perform job-related duties, which may require lifting, bending, stooping, stretching, pulling, and pushing loads up to 50 pounds or more as needed.
What We Offer

We offer a competitive salary of $55,000 - $65,000 per year, depending on experience, and excellent benefits, including Aetna Health Insurance, paid time off, and opportunities for professional growth and development.
