Responsibilities:- Conduct admissions and visits for our Home Care team, ensuring that patients receive the best possible care in the comfort of their own homes.
- Work independently and collaboratively as part of our interdisciplinary team to provide comprehensive care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide education and support to patients and their families to promote health and well-being.
- Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
- Graduate of an accredited school of nursing and licensed to practice as a Registered Nurse in the state of Minnesota.
- Minimum of 12 months experience working in an acute care or home health environment.
- Evidence of current CPR certification.
- Experience in Home Care, including OASIS, preferred.
- Able to travel within a geographic area serviced by the office and possess reliable transportation, a valid driver's license, and proof of auto insurance.
- Competitive annual salary range: $76,960 - $89,440.
- $6,000 hiring bonus.
- Mileage reimbursement and company car program.
