Key Responsibilities:

  • Provide home-based services to families, utilizing evidence-based practices to prevent out-of-home placements.
  • Collaborate with an interdisciplinary team to develop and implement individualized service plans.
  • Conduct regular home visits, assessments, and case management activities.
  • Develop and maintain relationships with community resources and service providers.
  • Participate in ongoing training and professional development to enhance skills and knowledge.

Requirements:

  • MSW or BA/BS in social work or related field.
  • Case management experience working with children and adolescents.
  • Crisis intervention and home-based experience a plus.
  • Valid driver's license.

Benefits:

  • 4 weeks paid vacation annually.
  • 13 paid holidays, 4 personal days.
  • 1 sick day per month, accruable to 150 days.
