Job Summary

We are seeking a skilled RN Case Manager to join our team in Finney County, Kansas. As a RN Case Manager, you will play a critical role in assessing, planning, and implementing care strategies for Medicare and Medicaid members. You will work closely with members to manage their health and healthcare, utilizing strong communication and interpersonal skills.

Key Responsibilities
  • Assess and plan care strategies for members with complex medical and behavioral needs
  • Develop and implement individualized care plans to promote health and well-being
  • Collaborate with interdisciplinary teams to ensure coordinated care and services
  • Communicate effectively with members, families, and healthcare providers to ensure quality care
  • Manage transitions of care for members discharged from in-patient hospital stays and emergency rooms
  • Identify and initiate referrals for social service programs and community resources
Requirements
  • Current, unrestricted RN license in the state of Kansas
  • 2 years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
  • Ability to travel up to 75% of the time with reliable transportation and a current, non-restricted state of Kansas driver's license and insurance
Preferred Qualifications
  • Bachelor's degree in nursing
  • Experience with managing Medicaid and Medicare populations with complex medical and behavioral needs
  • Experience with electronic charting
