Responsibilities

  • Communicate health and nursing needs to RN supervisors
  • Coordinate admissions, discharges, and follow-up care as needed
  • Assist with medical appointments and provide direct care as outlined in the Service Plan/Nursing Care Plan
  • Take and record vital signs, weights, wound measurements, and other essential metrics
  • Identify trends outside of parameters and communicate with RN/physician

Qualifications

  • Must be a Licensed Practical/Vocational Nurse (LPN or LVN) in good standing
  • CPR certification
  • At least six months of direct nursing experience
