At Transitions Hospice, we are committed to delivering exceptional patient care and support. As a Hospice Registered Nurse (RN), you will play a vital role in providing compassionate and holistic care to patients and their families.

Job Overview

The Hospice Registered Nurse (RN) is responsible for delivering nursing care to patients assigned to our program. This involves making regular visits to patients in their homes, where you will provide clinical and psychosocial support in accordance with the attending physician's orders and the inter-disciplinary team (IDT) plan of care.

This position requires excellent communication and interpersonal skills, as you will work closely with patients, families, physicians, and other healthcare professionals to ensure seamless care coordination.

Key Responsibilities:
  • Deliver high-quality nursing care to patients in their homes, addressing physical, emotional, and spiritual needs
  • Assess patient needs and develop individualized plans of care, incorporating data from various sources
  • Collaborate with the IDT to ensure comprehensive care and facilitate smooth transitions between care settings
  • Provide education and support to patients and families on disease management, self-care, and end-of-life issues
  • Document patient progress, medication regimens, and other relevant information in a timely and accurate manner
Requirements:
  • Current RN license in Indiana; minimum 1 year of experience in a clinical care setting or hospice environment preferred
  • Valid driver's license, reliable transportation, and valid insurance coverage for travel
  • Maintain up-to-date knowledge of regulations, policies, and procedures related to healthcare and hospice services
