Key Responsibilities:- Provide intermittent skilled nursing services to patients in their homes
- Communicate patient progress with other disciplines and direct, supervise, and instruct nonprofessional hospice aide staff in personal care
- Admit patients eligible for hospice services under physician's order
- Report patient status and need for other disciplines to clinical leadership, attending physician, and hospice physician
- Update care plans on an ongoing basis and complete informational visits and obtain patient consents for hospice admission
- Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
- Ability to assess patient needs and formulate individualized patient care plans
- Ability to work in a field setting and make sound nursing judgments
