Job Summary

We are seeking a highly skilled Care Manager to join our team at Centene Corporation. As a Care Manager, you will play a critical role in developing and implementing care plans that meet the unique needs of our members.

Key Responsibilities:

  • Assess the needs of our members and develop personalized care plans to ensure quality, cost-effective healthcare outcomes.
  • Coordinate with healthcare providers and community resources to ensure seamless care delivery.
  • Provide education and support to members and their families on available services and benefits.
  • Monitor care plans and make recommendations for improvement.
  • Collaborate with internal stakeholders to ensure compliance with regulatory requirements.

Requirements:

  • Bachelor's degree and 2-4 years of related experience, or equivalent experience acquired through accomplishments.
  • 3+ years of case management experience working with vulnerable populations with complex medical conditions.
  • Experience coordinating and managing medical services, and providing patient advocacy and education to Medicaid and STAR Plus Non-Waiver members.
  • Proficient computer skills and experience working within large databases or provider information systems and Microsoft Office applications.

Preferred Qualifications:

  • Experience in field-based LVN, LBSW, or LMSW Case Manager roles in home health, hospital, community health clinic settings.
  • Strong adaptability, flexibility, and resiliency skills.
