Nurse Practitioner

We are seeking a skilled and compassionate Nurse Practitioner to join our team at Chestnut Woods Rehabilitation & Healthcare Center. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the overall well-being of our residents.
Responsibilities- Deliver Patient-Centered Care: Provide individualized care to residents, adhering to established standards of nursing practice and physician-ordered treatment plans.
- Collaborate with Healthcare Team: Work closely with our Nurse Manager and other healthcare professionals to develop and implement effective care plans.
- Administer Medications and Treatments: Safely administer medications and treatments as prescribed by physicians, ensuring accurate documentation and adherence to protocols.
- Monitor Resident Conditions: Regularly assess and report on resident conditions, identifying potential issues and contributing to the development of individualized care plans.
- Document Patient Information: Accurately and thoroughly document patient information, including medical history, treatment plans, and progress notes.
- Active RN License: Possess an active, unencumbered license to practice as a Registered Nurse in Massachusetts.
- Covid Vaccination: Meet the requirement for Covid vaccination.
