Available Patient Placements in Various Locations:

- Implementing physicians' prescribed care plans
- Conducting patient assessments and coordinating care
- Administering prescribed medications, treatments, and therapies
- Maintaining compliant and timely documentation of care in the record system
- Promoting health and providing education and training to family members
- Accepting and documenting verbal orders from physicians as needed, ensuring co-signature from the Case Manager
- Following up with physicians, clients, family members, staff, and Case Managers regarding client issues, concerns, or changes in condition
- Performing other relevant duties as assigned
- Must possess and maintain an active, unencumbered license (RN/LPN/LVN) in the practicing state
- Current CPR certification from an accredited organization
- TB skin test (current within the last 12 months)
- Preferred six months of prior hands-on nursing experience
- Reliable transportation is required
