We are seeking a highly skilled and compassionate Part-Time Licensed Master Social Worker to join our team at Atlantic Dialysis Management Services. As a Social Worker, you will play a vital role in providing exceptional care and support to our patients and their families.
Responsibilities- Conduct thorough psycho-social assessments to determine the best course of treatment for each patient.
- Develop and implement individualized social work treatment plans in collaboration with the patient's medical team.
- Provide education and guidance to patients and their families on benefits available to them, including Medicare, Medicaid, and private insurance.
- Participate in multidisciplinary conferences to ensure comprehensive care and develop long-term care plans.
- Utilize crisis intervention techniques to address individual and family needs.
- Collaborate with community agencies to provide patients and their families with necessary resources and support.
- Liaise with affiliated hospitals and other dialysis centers to ensure seamless care and communication.
- At least 1 year of experience working with dialysis patients.
- Master's degree in social work with a specialization in clinical practice from an accredited School of Social Work.
- Certification from the New York State Education Department.
- Must be Licensed.
- Meet Federal regulations for Social Workers in ESRD Programs.
- $39.00 per hour.
- 22.50 hours per week.
