Job Title: Licensed Master Social Worker-Part-Time-Day Shift

We are seeking a highly skilled and compassionate Licensed Master Social Worker to join our team at Atlantic Dialysis Management Services LLC. As a key member of our dialysis center team, you will provide essential social case work and support to our patients and their families, ensuring they maintain a meaningful lifestyle despite the limitations of their illness and treatment.

Responsibilities:
  • Conduct thorough psycho-social assessments on each patient to determine the most effective social work treatment plan, aligned with the total medical treatment plan.
  • Stay up-to-date with knowledge of benefits available to patients under Public Law 92-603, Section 2991 for Medicare, as well as Medicaid, Blue Cross, and other insurance coverage.
  • Assist patients in understanding and obtaining the benefits to which they are entitled.
  • Participate in multi-disciplinary conferences and contribute to the development of long-term and short-term care plans for patients.
  • Utilize crisis intervention techniques to address individual and/or family needs.
  • Collaborate with psychiatric consultants as needed.
  • Refer patients and their families to relevant public and private community agencies, providing support in accessing transportation, public assistance, vocational rehabilitation, and other essential services.
  • Liaise between our dialysis center and affiliated hospitals, collecting and sharing pertinent information for patient transfers or visits to other hospitals or dialysis centers.
Requirements:
  • At least 1 year of experience working with dialysis patients.
  • Master's degree in social work with a specialization in clinical practice from an accredited School of Social Work and Certification from the New York State Education Department.
  • Must be Licensed.
  • Meet Federal regulations for Social Workers in ESRD Programs.

  • Competitive salary: $39.00 - $42.00 per hour.
