Job Summary

We are seeking a highly skilled and compassionate Direct Support Professional III LPN to join our team at the Health and Human Services Commission. As a key member of our staff, you will play a vital role in providing exceptional care and support to individuals with intellectual and developmental disabilities.

Key Responsibilities
  • Conduct competency-based training of staff on physical and nutritional management, oral and enteral eating, positioning, mobility, communication, and other related PNMP services.
  • Monitor programs, availability, condition, and proper use of assistive equipment, and ensure appropriate availability and condition of PNMP instructions and illustrations.
  • Attend PNMP meetings and clinics, NMT meetings, and PST/HST meetings as requested, and provide training to Direct Support Staff on changes in PNMPs, positioning, lifting, transferring, and meal time practices.
  • Monitor lifting, positioning, transfers, walking programs, use of communication devices, and mealtime supports, and report problems to the appropriate home supervisors, Unit administration, and Habilitation Therapies.
