Job OverviewThis is an exciting opportunity to leverage your expertise and compassion as a Nurse Practitioner or Physician Assistant to make a direct impact on the health and well-being of others. As a PRN/Part Time provider, you will have the flexibility to choose your schedule and work in various settings, including home visits, skilled facilities, and telehealth.
Key Responsibilities- Conduct thorough adult and geriatric assessments, medical history evaluations, diagnoses, treatment plans, health education, physician referrals, case management referrals, follow-up care, and clear documentation according to Matrix guidelines and protocols.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed to provide comprehensive care.
- Work closely with Primary Care Physicians (PCPs) to educate patients, provide follow-up care, and ensure seamless transitions between care settings.
- Provide services in diverse settings, including home visits, skilled facility visits, and telehealth visits.
- A Master's Degree in Nursing or a related field, or equivalent experience and satisfactory completion of NP licensure requirements.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by a recognized organization, such as the NCCPA, AANP, ANCC, or AACCN, in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant, with the ability to travel within the state or out of state.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills, with the ability to effectively interact with patients, clients, and colleagues.
- A positive and outgoing personality, with a strong commitment to teamwork and adaptability.
- A valid state driver's license and reliable transportation, with proof of adequate automobile insurance coverage for the state of residence.
- The ability to travel to meet patients in their homes, as required.
