About the PositionThe role involves conducting comprehensive health assessments in the home or other community environments, reviewing and observing a member's current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member's overall health and well-being that can be difficult to capture during routine office visits.
The assessment typically lasts up to one hour and involves working collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.
Responsibilities- Conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as necessary.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up as needed.
- Provide services in various venues, including Home Visits, Skilled Facility Visits, and Telehealth Visits.
- A Master's Degree or equivalent experience and satisfactory completion of NP licensure.
- Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
- A minimum of 1 year of experience as a Nurse Practitioner or Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- A positive and outgoing personality, with a willingness to be flexible and adaptable.
This is a PRN/Part-Time position with a competitive compensation package, including a salary of $110.00 per completed assessment, mileage reimbursement, and opportunities for career growth and development.
