About the Position
As a Home Risk Assessment Clinician with Matrix Medical Network, you will conduct comprehensive health assessments, reviewing and observing patients' current health, medical history, medication adherence, social environment, and other risks. This provides valuable insight into patients' overall health and well-being, improving quality of care and potentially closing multiple care gaps with a single visit.

Responsibilities
  • Conduct adult/geriatric assessments, medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as needed.
  • Collaborate with Primary Care Physicians on patient education and provide follow-up.
  • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.

Qualifications
  • Must have a Master's degree or equivalent experience and satisfactory completion of NP licensure.
  • Must have current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Must be board certified by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Must have current BLS, ACLS, or CPR certification.
  • Must have a minimum of 1 year experience as a Nurse Practitioner/Physician Assistant and be able to travel either in-state or out-of-state.
  • Must have strong computer skills and familiarity with EMR software.
  • Must possess excellent verbal and written communication skills and a positive, outgoing personality.
