Job Summary

Become a valued member of our team at CenterWell Home Health and play a vital role in providing professional rehabilitation services to patients in the comfort of their own homes.

As a Physical Therapist Assistant, you will work closely with our Physical Therapist to administer treatment plans, provide manual therapeutic exercises, and modalities to patients. Your attention to detail and excellent communication skills will be essential in accurately documenting and submitting clinical paperwork in a timely manner.

Key Responsibilities
  • Administer physical therapy treatment plans developed by the Physical Therapist.
  • Provide non-complex active and passive manual therapeutic exercises, therapeutic massage, and modalities to patients.
  • Accurately document and submit clinical paperwork in a timely manner.
  • Attend educational meetings and coordinate department in-services.
  • Notify supervisor of any equipment failures, lack of supplies, and client changes.
Requirements
  • Associate degree in Physical Therapy and current state license.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Proof of current CPR certification.
  • Minimum one year experience as a Physical Therapy Assistant; Home Health experience a plus.
  • Comprehensive knowledge of the geriatric population.
  • Good verbal and written communication and organization skills.
  • Self-directed, enthusiastic, and accepts constructive feedback.
Benefits

  • Competitive pay range of $57,700 - $79,500 per year.
