About the Job

Pinnacle Home Care is seeking a skilled Physical Therapist to provide high-quality care to our geriatric patients in the Palm Beach Gardens, FL area. As a Physical Therapist, you will play a vital role in helping our patients achieve their rehabilitation goals and improve their overall quality of life.

Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop personalized treatment plans.
  • Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions.
  • Collaborate with other healthcare professionals to ensure seamless care and achieve optimal patient outcomes.
  • Communicate effectively with patients, families, and caregivers to ensure understanding and compliance with treatment plans.
  • Document patient progress and maintain accurate records.
Qualifications:
  • Graduate of an accredited Physical Therapy program.
  • Licensed to practice as a PT in the State of Florida.
  • Current BLS Certification.
  • At least one year of experience as a Physical Therapist.
  • Home healthcare experience preferred but not essential.
