We are seeking a highly motivated and compassionate Care Navigator to join our team at AIDS Healthcare Foundation. As a Care Navigator, you will play a critical role in coordinating and navigating medical and social services to support individuals in need.
Key Responsibilities- Engage and Support High-Risk Populations: Work with vulnerable patient populations, including those with substance abuse issues, to provide counseling, motivation, and support.
- Community Outreach and Engagement: Engage with the community to locate individuals who require healthcare services but are not currently accessing or adhering to them.
- Assessment and Planning: Conduct assessments to identify clients' strengths, needs, abilities, and preferences, and develop individual plans of care and/or transition plans accordingly.
- Linkage and Referral Services: Arrange for services through linkage, referral, and navigation services to ensure clients receive necessary support.
- Education and Support: Provide HIV, STD, medical adherence, and risk reduction education to individuals, as well as guide them in applying for and accessing eligible benefits.
- Group Facilitation: Prepare and facilitate groups on life skills, social rehabilitation, and psycho-education.
- Communication and Coordination: Support clients in communicating with external and internal providers, and meet with interdisciplinary treatment team members to coordinate client services and document outcomes.
- Documentation and Productivity: Submit all documentation in an accurate and timely manner, and achieve and maintain program-established productivity levels.
- Experience: Two years of experience in social services or equivalent experience, with a high school diploma or GED equivalent.
- Knowledge and Skills: Knowledge of HIV/AIDS and conducting intervention/prevention services with minority and at-risk populations, effective verbal and written communication skills, and the ability to work in a stressful environment with high-risk, vulnerable clients.
- Certifications and Training: HIV 500/501 certification, SOAR training, and a valid Florida driver's license with ongoing proof of insurance.
