LPN Ambulatory Care Specialist

Job Summary
SightMD is a rapidly growing integrated ophthalmic provider group, extending the reach of exceptional eye care to patients across the region.
As a Licensed Practical Nurse in our Ambulatory Care team, you will play a vital role in delivering high-quality nursing care to patients in our clinics.
Key Responsibilities
- Provide individualized patient care, according to the dependent and independent nursing functions.
- Utilize nursing techniques, procedures, and established standards based on the scope of practical nursing, under the direction/delegation of the registered nurse.
- Maintain and coordinate communication among patient/significant other, staff and physician as evidenced within medical record, client, and health care team feedback.
- Give punctual change of shift report consisting of pertinent and relevant information for assigned patients.
- Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by another department feedback.
- Answer and refer telephone calls or other inquiries to ensure accurate and timely communications are facilitated.
Requirements
- Graduate of an accredited school of Nursing.
- Valid registration as an LPN in the state of Pennsylvania.
- Completion of relevant state Board of Nursing approved.
- Current AHA Healthcare Provider training (BCLS).
