Join Our Team as a Licensed Practical Nurse

Affirmed Home Care is a leading provider of home healthcare services, and we are seeking a skilled Licensed Practical Nurse to join our team.

Responsibilities:

  • Provide skilled nursing care to patients in their homes
  • Conduct thorough assessments and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patients' vital signs and report any changes to the Director of Clinical Nursing
  • Document patient care and progress in a timely and accurate manner

Requirements:

  • Valid LPN license
  • Annual physical within one year
  • PPD or Quantiferon within one year or chest x-ray within five years
  • MMR labs
  • COVID vaccine card with booster
