Position Overview

This role operates under the guidance of the Nurse Manager or Clinical Coordinator. The Clinical Nurse Specialist (CNS) is responsible for assessing, planning, implementing, evaluating, and supervising individualized patient care within a designated nursing unit or department, adhering to established policies and procedures.

Additionally, this position includes overseeing non-registered nursing personnel on the assigned unit. When in a leadership capacity, the CNS will delegate and supervise unit activities. Flexibility to work across various nursing units may be required based on staff and patient distribution. Other responsibilities may be assigned to ensure a high standard of service delivery.


Key Responsibilities

  • 1. Continuously assess the physical and psychosocial conditions of patients throughout their care journey.
  • 2. Develop, communicate, and document a personalized care plan that addresses individual patient needs and interventions, involving the patient, family, and healthcare team in the process.
  • 3. Implement and coordinate the care plan in collaboration with the healthcare team.
  • 4. Evaluate the effectiveness of the care plan, the patient's response to treatment, and readiness for discharge, making necessary adjustments and referrals as needed.
  • 5. Plan educational initiatives based on the assessed learning needs and readiness of patients and their families, providing educational opportunities and documenting outcomes.
  • 6. Serve as a mentor to new staff members.
  • 7. Assume charge nurse responsibilities as required.
  • 8. Meet performance expectations related to customer service, teamwork, resource utilization, and personal and professional development as outlined in performance evaluations.
  • 9. Perform additional duties as assigned to ensure the smooth operation of the department or unit.

Clinical Competencies
Demonstrate proficiency and understanding of procedures and protocols associated with the care of patients requiring:

  • Arterial lines, cardiac monitors, central venous pressure, and Swan-Ganz catheters, including cardiac output.
  • Arrhythmia interpretation and appropriate interventions, including troubleshooting cardiac equipment.
  • Nursing responsibilities and interventions for patients on ventilators, CPAP, PRVC, and PEEP, including care for tracheostomy or endotracheal tubes according to established protocols.
  • Assistance with intubation and extubation procedures as per established guidelines.
  • Defibrillation and cardioversion techniques.
  • Pre and post-operative care for patients with temporary or permanent pacemakers, including care for patients requiring external pacemakers.
  • Operation of equipment and care for patients with intra-aortic balloon pumps (IABP).
  • Competency in patient preparation, medication administration, and follow-up for patients receiving thrombolytic agents.
Qualifications

Education
Graduate of an accredited nursing program; a Bachelor of Science in Nursing (BSN) is preferred. Proficiency in computer software, including electronic patient records, is essential. Ability to conduct literature searches online regarding patient care is required.

Experience
A minimum of two years of experience in medical-surgical nursing is preferred, with critical care experience being advantageous.

Licensure
Must possess a valid Registered Nurse license in the state of Connecticut.

Special Skills
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications are required within the first year of employment.
