As a Home Health RN with Optimum Staffing, Inc., you will be responsible for assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.



Job Summary:


  • Deliver patient care within organizational philosophy, policy, and standards of community health practice.
  • Perform admission processes and case management responsibilities in accordance with MGB Home Care's policies and standards of practice.
  • Utilize thorough and timely electronic documentation and interdisciplinary communications.


Responsibilities:

Assess patient conditions, implement care plans, and provide ongoing education and support to patients and their families.



Deliver patient care within organizational philosophy, policy, and standards of community health practice.



Perform admission processes and case management responsibilities in accordance with MGB Home Care's policies and standards of practice.



Requirements:


  • 1 year Traveler and 1 year Home Care experience.
  • 1 year Case Management Experience.
  • Ability to travel to cases as assigned.


Compensation:

$60.00 - $65.00 per hour
