**Job Summary**

We are seeking an experienced Home Health Physician - Primary Care Specialist to join our team. As a primary care physician, you will provide medical care and treatment services to patients in their homes, with a focus on chronic disease management and palliative care. You will work closely with our multidisciplinary team to develop and implement comprehensive care plans that address the physical, emotional, and social needs of our patients.

**Responsibilities**

* Provide medical consultation and support to non-physician providers as needed* Conduct thorough medical histories, physical exams, and diagnostic tests to diagnose and treat acute and chronic medical conditions* Develop and implement proactive patient-centered care plans in collaboration with other healthcare professionals* Prescribe and dispense medications and drugs to suitable patients* Chart treatment records and monitor results* Educate patients and families on goal-based preventative care, chronic care, or urgent care needs, as well as the use of prescribed medications and treatment* Participate in quality improvement activities and contribute to the development of policies and procedures* Maintain accurate and timely medical records according to agency protocols

**Requirements**

* Licensed physician (MD or DO) in the State of Oregon* Completion of internship and residency in Internal Medicine or Family Medicine* Certification by the American Board of Medical Specialties* DEA license* Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle* CPR certification at hire or within 6 months in position* Geriatric and/or palliative care certified preferred* Experience in medical home visits preferred

**Work Environment**

* Work in a fast-paced, dynamic environment with a multidisciplinary team* Interact with diverse patients, staff, and external partners* Spend time outside of the office, visiting patients in their homes* Use electronic systems to document patient information, communicate with colleagues, and stay up-to-date on industry developments* Meet productivity metrics while maintaining high-quality patient care* Participate in regular team meetings and training sessions

**Compensation & Benefits**
