As a **Care Transitions Coordinator Home Health**, you will play a critical role in navigating patients through post-acute care, promoting quality and efficiency in the delivery of services.
We offer a competitive salary range of $65,000 - $85,000 per year, based on experience and qualifications.
Responsibilities:
- Assist patients in transitioning from acute care to post-acute care settings.
- Develop patient-centered transition plans using evidence-based clinical guidelines.
- Represent Enhabit Home Health & Hospice in strategic relationships with healthcare partners.
Requirements:
- Registered Nurse (RN) or Physical Therapist (PT) licensure in good standing.
- Two years' demonstrated field experience in home health or hospice.
- Valid state driver's license and automobile liability insurance.
