Job Description

This is an exciting opportunity for a goal-driven Registered Nurse (RN) or Physical Therapist (PT) to join our team as a Care Transitions Specialist. As a key member of our post-acute care team, you will be responsible for assisting patients in navigating the transition from hospital to home, ensuring a smooth and seamless experience.

Responsibilities
  • Assist patients in developing personalized transition plans to promote quality and efficiency in post-acute care
  • Represent the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups
  • Collaborate with cross-functional teams to ensure seamless transitions and high-quality patient outcomes
