Role Overview
This role requires a dedicated and empathetic individual to work as a Care Specialist within our organization. As a Care Specialist, you will be responsible for delivering high-quality care to our patients, primarily in their homes and communities, with occasional virtual interactions.
Key Responsibilities
- Patient Outreach: Conduct direct outreach to patients via phone, in-person visits, mailings, and other strategies, including cold-calling and unscheduled door knocks.
- Enrollment and Data Collection: Enroll patients into our program and collect essential data about them during the enrollment process through continuous phone and in-person interactions.
- Needs Assessment and Goal Setting: Meet patients in their homes or communities to conduct a thorough needs assessment, helping them set achievable health goals.
- Care Coordination: Facilitate in-home provider appointments and coordinate care between patients and our care team as needed, ensuring timely initial provider visits and follow-up appointments.
- Quality Measure Support: Support patients in meeting their healthcare goals related to quality measures.
- Interdisciplinary Teamwork: Collaborate with our interdisciplinary team to support market and organizational goals.
- Data Analysis and Documentation: Analyze patient data to determine patient needs or treatment goals, obtain and record vital signs, and document patient encounters accurately.
- Education and Communication: Explain technical medical information to educate patients and cultivate strong relationships with primary care providers through timely communication and integration of information into the care plan.
- Rapport Building and Extended Appointments: Build rapport with patients using motivational interviewing techniques and conduct one-on-one extended in-person appointments.
- Follow-up and Reporting: Make follow-up calls and home visits per Upward Health policy, prepare reports as needed, and document each patient encounter with precision.
Required Skills and Qualifications
- Critical Thinking and Problem-Solving: Strong critical thinking skills for assessing patient needs and treatment goals, with the ability to work independently in an unstructured environment.
- Interpersonal Savvy and Motivational Interviewing: Excellent interpersonal skills, demonstrated by the ability to interact with and influence people to establish trust and build strong relationships, with familiarity with concepts like motivational interviewing and trauma-informed care.
- Organization and Time Management: Strong organizational skills, ability to manage and maintain a personal schedule, and proficiency in time management and prioritization.
- Communication and Documentation: Excellent communication skills, with the ability to explain technical medical information and accurately document patient information in electronic medical records.
- Qualifications: At least 2 years of relevant work experience as a Community Health Worker, Peer Support Specialist, Medical Assistant, or similar role, with a high school diploma or equivalent required.
- Additional Requirements: A valid driver's license, auto liability insurance, reliable transportation, and the ability to travel within your assigned territory or other areas as needed to support patient needs and training requirements.
Salary: $65,000 - $80,000 per year, depending on location and experience.
