The Outreach Specialist Role

We're seeking a skilled and compassionate Healthcare Community Liaison - Patient Advocate to join our team. As a key member of our outreach team, you'll be the first point of contact for our patients, responsible for making an excellent first impression and effectively communicating our service offering. Your primary objective will be to inform patients about our services and help them get started on their healthcare journey.

You'll manage a caseload of patients, utilizing various strategies to connect with potential patients and engage those already enrolled in our program. This may involve phone calls, in-person meetings, mailings, and other tactics. You'll also be responsible for enrolling patients into our program, collecting key data during the enrollment process, and interacting with enrolled patients as needed to ensure ongoing engagement or deliver additional community-based touchpoints.

Key Responsibilities

  • Conduct direct outreach to patients via phone, in-person meetings, mailings, and other strategies.
  • Communicate with patients about the role that Upward Health can play in helping them improve their health.
  • Enroll patients into Upward Health's program and collect key data about patients during the enrollment process.
  • Manage a personal caseload of patients and ensure that each assigned patient is contacted using the most effective strategy.
  • Interact with enrolled patients as needed to ensure ongoing engagement or deliver additional community-based touchpoints.
  • Participate in weekly team meetings focused on ongoing education and improvement.
  • Accurately and timely document interactions in our company's software system.

Requirements

Education and Experience

Candidates should have a minimum of 3 years of work experience, preferably in sales, healthcare, or outreach roles. A background in community resources and knowledge of local markets is also beneficial. Fluency in English is required; Spanish proficiency is a plus. Reliable transportation is necessary for essential outreach functions.

Salary Range

$60,000 - $80,000 per year, depending on experience.
