Job Summary

The Human Trafficking Patient Navigator Specialist will provide critical support to adult and child victims of human trafficking within the healthcare setting. This role involves intake, case management, and coordination of services to ensure comprehensive care and support.

Key Responsibilities
  • Provide direct services such as intake, navigation, education, and follow-up services for those identified as having current or former trafficking experiences.
  • Assist in data collection and reporting of data indicators required by the SOAR Demonstration Program.
  • Coordinate medical services with clinic and behavioral health personnel.
  • Assist in community outreach, universal education, screening, safety planning, and risk reduction.
  • Collaborate with community partners and social service agencies to obtain services for patients and make referrals.

Requirements

The ideal candidate will possess a Bachelor's or Master's degree in a social science field, such as social work, psychology, or public health. Experience working in behavioral health, mental health, child welfare, or a related profession is required. Bilingual applicants with a Master's degree and crisis intervention training are preferred.
