Aveanna Healthcare is seeking a skilled and compassionate Home Health Nurse RN to join our team. As a Private Duty Nurse RN, you will provide high-quality patient care to adult patients in their homes, promoting independence and improving their quality of life.
Responsibilities
- Implement and manage patient care plans in collaboration with physicians and other healthcare professionals.
- Conduct thorough patient assessments and develop individualized care plans to meet their unique needs.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Document patient care and progress in a timely and accurate manner.
- Provide education and training to patients and their families on disease management, medication use, and other health-related topics.
- Collaborate with the care team to ensure seamless transitions of care and optimal patient outcomes.
Requirements
- Current RN licensure in the state of practice.
- At least six months of prior nursing experience, preferably in home health or a related setting.
- Current CPR certification and TB skin test within the last 12 months.
- Reliable transportation and a valid driver's license.
